Procedures for Technical Assistance or Service Return Authorization
Before calling please have the following information at your disposal....
- Your Name and Company Name.
- The defective equipment (or the equipment model part number and serial number).
- A complete description of the problem.
- A contact name, phone number and Email address.
- A return shipping address.
Call Premier Computer Services at (309) 764-7123 or (800) 322-7123....
- When the receptionist answers, give her your name and the name of your company.
- Ask to speak to the Customer Service Department.
- All information regarding your repair will be maintained in our records under the specific SO number.
- Please, Do Not reuse an "old" SO number.
- Please read our Disclaimer prior to sending in your equipment

Disclaimer and shipping instructions
Please follow the instructions before sending in your computer for repair.
- BACKUP ALL DATA TO AN EXTERNAL BACKUP DEVICE.
- YOUR DATA AND PROGRAMS MAY BE LOST OR UNUSABLE DUE TO THE REPAIR PROCESS.
- It is the customers responsibility to reinstall all programs and data.
- Premier Computer Services, Inc is not responsible for data or program loss.
- All repair and shipping charges must be paid in full before computer is returned.
- A diagnostic and shipping fee will apply to all repairs not completed.
- Record your model and serial number in a safe place.
- Include a list of items shipped.
- Include contact information; a name, phone number, email address and return mailing address.
- Include a detailed description of the problem your computer is having.
- Include any special repair or return shipment instructions.
- Use bubble wrap or other suitable packing material and allow at least four inches of packing material around the computer.
- Write the Service Order number given by customer service on the outside of the box.
- Send your computer to: Premier Computer Services Inc., 3003 48th Ave., Moline, IL. 61265.
- Please make sure you have completed all of the above instructions before sending in your computer for repair!
